Learning Objectives:
1. Review the evolution of the patient-centered medical home (PCMH) model of care and its applicability in today's health care environment.
2. Synthesize the NCQA PCMH standards with health center operations.
3. Create an action plan to bring your health center into alignment with standards for patient-centered care.
Session Description:
Review the 2017 NCQA PCMH standards and demonstrate how each fits into health center operations. The standards include team-based care and practice organization, knowing and managing your patients, patient-centered access and continuity, care management and support, care coordination and care transitions, and performance measurement and quality improvement. Action planning tools will be provided to help the attendees document deficiencies and potential action steps to improve perfomance.
